Jack Gerlitz

Jack Gerlitz is a Doctor of Physical Therapy and the founder of Uplift Physical Therapy and Wellness in Midlothian, VA. A Delaware native who has called the Richmond area home for over a decade, Jack earned his B.S. from James Madison University and his Doctorate in Physical Therapy from Virginia Commonwealth University. After working across outpatient private practice, rural hospital-based, and corporate PT settings, he saw the same problem everywhere: the system wasn't putting patients first. He launched Uplift to change that.
Jack specializes in the treatment of cervical pain, headaches, and temporomandibular joint dysfunction (TMD), and works closely with local dentists to provide integrated, interprofessional care. He also has a strong clinical focus in vestibular disorders and concussion management — an interest shaped in part by his own personal experience with vertigo and chronic dizziness. Throughout his career he has treated a wide range of patients, including active adults, post-surgical patients, runners, and older adults.
His approach is built around finding root causes, building meaningful patient relationships, and giving people the tools to resolve their problems — even when other approaches have fallen short. Outside of the clinic, Jack enjoys trail running, golf, and traveling with his wife Kristen and their dog Milo.

What makes Uplift different from a regular PT clinic?
At Uplift, every session is one-on-one with your Doctor of Physical Therapy — no aides, no assistants, and no 30-minute appointments shared with three other patients. Sessions are 60 minutes, and the initial evaluation is 90 minutes. The goal is to identify what is driving your symptoms, not just manage them temporarily.
Do you accept insurance?
Uplift is out of network with commercial insurance plans and operates as a fee-for-service practice. We do accept Traditional Medicare and Medicare Supplemental Plans, and HSA/FSA payments are accepted. For all other patients, we can provide a superbill at the conclusion of services for potential out-of-network reimbursement through your plan.
Can PT actually help with jaw pain or headaches?
Many people come to Uplift with jaw pain, chronic headaches, or craniofacial discomfort after trying other approaches without resolution. We work closely with local dentists when integrated care is appropriate and conduct a thorough evaluation to determine the underlying drivers of symptoms and the right path forward.
What should I expect when I first reach out?
The first step is a free 15-minute phone consult with our team to make sure Uplift is a good fit for your situation. From there, you'll schedule a 90-minute in-person evaluation to identify the root cause of what you're dealing with. Follow-up sessions are 60 minutes, one-on-one.
Do you offer telehealth?
Not at this time. All care at Uplift is delivered in person at our Midlothian, VA location inside the Rockwell Physicians Building in Salisbury. Hands-on treatment is central to how we work, and that requires being in the room together. Mobile visits are offered based on patients condition and scheduling availability.
